چکیده

Prior studies have demonstrated the safety of same day discharge in appropriately selected patients undergoing CIED implantation though a limitation is requirement for prolonged post-operative observation. There limited data on whether expedited pathways, which can reduce resource utilization, offer comparably safe outcomes. To evaluate outcomes an (SDD) protocol across hospitals. All pacemaker and ICD between 2018-2021 at three hospitals were identified. The pathway SDD included two-hour (1) PA/lateral chest x-ray (2) device interrogation including set up remote monitoring (3) bedside evaluation. This was implemented over study period use physician discretion. Patients’ sociodemographics, comorbidities, procedure retrospectively collected. Outcomes 30-days post-implant unplanned healthcare utilization (urgent care, emergency department, or hospital visits) as well 90-day infections. Differences versus not assessed using chi-square test categorical variables t-test continuous variables. Multivariate logistic regression analysis used to association grouping interest. Between 2018-2021, 4872 underwent 28.3% discharged day. As summarized Table 1, compared SDD, older, more likely be female, had higher incidence comorbidities hypertension, diabetes mellitus, stroke, heart failure. Among day, mean time post-operatively 2.8 (SD±1.8) hours. Adjusting baseline characteristics, signficantly lower 30-day (AOR: 0.49 95%CI: 0.40-0.59), complications 0.60 0.38-0.93) mortality 0.19 0.06-0.62). Additionally, infection rates significantly different based grouping. An evaluation appears without increase adverse events hospitalized extended Further investigation define patient selection criteria warranted.

Oral anticoagulation management in patients with atrial fibrillation undergoing cardiac implantable electronic device implantation

BACKGROUND Oral anticoagulation (OAC) therapy is associated with increased periprocedural risks after cardiac implantable electronic device (CIED) implantation. Patterns of anticoagulation management involving non-vitamin K antagonist oral anticoagulants (NOACs) have not been characterized. HYPOTHESIS Anticoagulation strategies and outcomes differ by anticoagulant type in patients undergoing ...

Morphometric parameters of cardiac implantable electronic device (CIED) pocket walls observed on device replacement.

BACKGROUND The final stage of a conventional de-novo cardiac implantable electronic device (CIED) implantation procedure with transvenous lead insertion involves the formation of a pocket by tissue separation superficial to the pectoralis major muscle in the right or left infraclavicular region, where the device is subsequently placed. Over time, a scar "capsule" forms around the CIED as a resu...

Abstract: Same-Day Discharge for Women Undergoing Implant-Based Breast Reconstruction Using an Enhanced Recovery after Surgery Model Is Safe

P oters with implants in female 18 years old or more; 2 the use of intraoperative irrigation with antibiotic; and 3 documentation of capsular contracture. Our primary outcome was postoperative rate of severe capsular contracture (Baker III and IV). We assessed the methodologic quality of included studies using validated tools (JADAD and MINORS). Pooled random effects estimates and 95% confidenc...

Dosimetric review of cardiac implantable electronic device patients receiving radiotherapy

A formal communication process was established and evaluated for the management of patients with cardiac implantable electronic devices (CIEDs) receiving radiation therapy (RT). Methods to estimate dose to the CIED were evaluated for their appropriateness in the management of these patients. A retrospective, institutional review board (IRB) approved study of 69 patients with CIEDs treated with ...
